CONTRAT DE SERVICES – LE PANSA PUP CLUB

Entre :
Le Pansa Pup Club, entreprise offrant des services de garderie canine
(ci-après « l’Établissement »)

Et :
Le propriétaire du chien
(ci-après « le Client »)

1. OBJET DU CONTRAT

Le présent contrat encadre les services de garderie, d’évaluation comportementale et de soins offerts par Le Pansa Pup Club.

2. CONDITIONS D’ADMISSION

Le Client confirme que son chien :

  • Est en bonne santé

  • Est à jour dans ses vaccins (preuve exigée)

  • Est sociable avec les humains et autres chiens

  • N’est pas agressif

  • N’est pas une femelle en chaleur

Le Pansa Pup Club se réserve le droit de refuser ou retirer un chien à tout moment pour des raisons de sécurité.

3. RESPONSABILITÉS DU CLIENT

Le Client s’engage à :

  • Fournir des informations exactes et complètes

  • Divulguer tout problème médical ou comportemental

  • Assumer les frais vétérinaires en cas d’urgence

4. RISQUES ET DÉCHARGE DE RESPONSABILITÉ

Le Client reconnaît que les activités canines comportent des risques inhérents.

Le Client dégage Le Pansa Pup Club, ses employés et partenaires de toute responsabilité en cas de :

  • Blessures mineures

  • Stress ou maladie

  • Incidents imprévisibles entre chiens

Sauf en cas de négligence grave démontrée.

5. URGENCES MÉDICALES

En cas d’urgence, Le Pansa Pup Club est autorisé à :

  • Contacter un vétérinaire

  • Prendre toute décision nécessaire au bien-être du chien

Les frais sont à la charge du Client.

6. POLITIQUE D’ANNULATION

  • Annulation minimum 24h à l’avance

  • Absence ou annulation tardive : frais applicables

  • Aucun remboursement pour services entamés

7. PHOTOS ET VIDÉOS

Sauf avis contraire écrit, le Client autorise l’utilisation de photos/vidéos de son chien à des fins promotionnelles.

8. PROTECTION DES RENSEIGNEMENTS PERSONNELS

Les renseignements sont traités conformément à la Loi 25 (Québec).
Le Client peut demander l’accès ou la suppression de ses données.

9. ACCEPTATION

En utilisant les services de Le Pansa Pup Club, le Client confirme avoir lu, compris et accepté ce contrat.

SERVICE AGREEMENT – LE PANSA PUP CLUB
Between:
Le Pansa Pup Club, a company providing dog daycare and related services
(hereinafter referred to as the “Facility”)

And:
The dog owner
(hereinafter referred to as the “Client”)

1. PURPOSE OF THE AGREEMENT

This agreement governs the dog daycare, behavioral assessment, and care services provided by Le Pansa Pup Club.

2. ADMISSION REQUIREMENTS

The Client confirms that their dog:

  • Is in good health

  • Is up to date on all required vaccinations (proof required)

  • Is social with humans and other dogs

  • Has no history of aggression

  • Is not a female in heat

Le Pansa Pup Club reserves the right to refuse or remove any dog at any time for safety or behavioral reasons.

3. CLIENT RESPONSIBILITIES

The Client agrees to:

  • Provide accurate and complete information

  • Disclose any medical or behavioral conditions

  • Assume all veterinary costs in the event of an emergency

4. RISKS AND RELEASE OF LIABILITY

The Client acknowledges that canine daycare activities involve inherent risks.

The Client releases Le Pansa Pup Club, its employees, and partners from any liability related to:

  • Minor injuries

  • Stress or illness

  • Unpredictable incidents between dogs

Except in cases of proven gross negligence.

5. MEDICAL EMERGENCIES

In the event of a medical emergency, Le Pansa Pup Club is authorized to:

  • Contact a veterinarian

  • Make any necessary decisions to ensure the dog’s well-being

All related costs are the responsibility of the Client.

6. CANCELLATION POLICY

  • Cancellations must be made at least 24 hours in advance

  • Late cancellations or no-shows may be subject to fees

  • No refunds for services already rendered or initiated

7. PHOTOS AND VIDEOS

Unless otherwise stated in writing, the Client authorizes Le Pansa Pup Club to use photos and videos of their dog for promotional and marketing purposes.

8. PERSONAL INFORMATION PROTECTION

Personal information is handled in accordance with Quebec’s Law 25.
The Client may request access to or deletion of their personal data at any time.

9. ACCEPTANCE

By using the services of Le Pansa Pup Club, the Client confirms that they have read, understood, and accepted this Service Agreement.
